Troubleshoots any barriers the patient might have in adhering to care.
Engage in the community through outreach to recruit and engage high-risk patients.
Serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
Assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease.
Build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as community education, social support and advocacy.
Collaborate with patients and their care teams to create solutions to overcome barriers to achieving healthcare goals.
Connect patients with community resources and link patients with health center services as appropriate.
Follow-up with patients via phone calls, home visits and visits to other settings where patients can be reached.
Performs general support activities and carries out special projects as required.
conduct public education activities to raise awareness of the availability of qualified health plans (QHPs) distribute fair and impartial information concerning enrollment in QHPs facilitate enrollment in QHPs by assisting individuals in reviewing their health coverage options as well as complete eligibility and enrollment forms provide information in a manner that is culturally and linguistically appropriate to the needs of the population being served by the marketplace.

Knowledge, Skills, and Abilities: Knowledge and competencies with harm reduction, motivational interviewing and relevant treatment modalities that are being used in the field.
Knowledge of culture and values in the local community.
Knowledge of principles and processes for providing customer and personal services.
This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
Ability to give full attention to what other people are saying, taking time to understand the points being.
Ability to communicate information and ideas in speaking so others will understand.
Proficient in Microsoft Excel.
Excellent verbal and written communication skills.
Experience and Education Qualifications: Bachelor’s degree from accredited college or university in related field.
Two (2) years of experience working in community health, human services outreach and/or healthcare preferred.
Desirable experience includes: customer service, patient/peer coaching, medical patient interaction, community relations, outreach, data entry and public speaking.
Supervisory Requirements: None Employment Requirements: Must provide evidence that incumbent is free of infectious and contagious disease prior to beginning employment and annually thereafter.
TB– Expected for all direct care staff.
Mandatory for Kansas Residential staff.
Annual Health Assessment- Mandatory Kansas Youth Residential staff.
Successful completion of the Criminal Record and Child Abuse & Neglect Background check prior to beginning employment.
Completion of New Hire Orientation within 30 Days of employment.
All training requirements including Relias Learning within two (2) weeks of employment and annually thereafter.
Achievement of the following training.
Missouri Staff: thirty-six (36) hour training requirements within 2 year period in accordance with 9 CSR 10-7.
110 E 1.
Kansas Residential Youth Homes: eighteen (18) hours of training annually.
First Aid certification within three (3) months of beginning employment and upon renewal date.
Obtain CPR certification within three (3) months of beginning employment and upon renewal date.
Behavioral Management training completed within thirty (30) days of employment or first available class after start date: CPI for Residential programs Peaceful Intervention or Verbal De-escalation for Outpatient programs Completion of “Direct Service Expectations” within 30 Days of employment.
Extensive knowledge of the policies, procedures, and regulations of the program to which the employee is assigned.
Valid Driver’s License in state of residence.
Physical Requirements: ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up the 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body.
Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time.
Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
FOR FQHC (FOSTER CARE) Essential Job Functions: Assist foster parents in accessing primary medical, dental, and behavioral health care for children in their care with a focus on getting children seen for required initial and follow up visits Conduct SDOH screening utilizing the PRAPARE Assessments to determine SDOH needs of children and foster families annually or more frequently, if change in foster child placement Provide care coordination, which may include but is not limited to facilitating care transitions, supporting the completion of referrals, and providing or confirming appropriate follow-up.
Make referrals and connections to community resources to help individuals and families meet basic social needs.
Other duties as assigned.

Experience and Education Qualifications: PFH/Clarity will strive to employ a CHW with “lived experience”: CHW who was a child in Missouri’s foster care program.
CHW who is/was a parent in Missouri’s foster care program.
CHW who has adopted a child from a foster care program.
CHW who has worked for and/or volunteered with a child welfare, child advocacy, juvenile justice, or behavioral health organization.
Undergraduate Bachelor’s degree in Social Work, Psychology, Public Health Nursing, Health Education, or a related field from an accredited school.
Two (2) years of experience working in community health, human services outreach and/or healthcare preferred.
